The new patient registration process is easy and simple.
Note: if your company is part of the Optum Behavioral Solutions Preferred Group program please click here.
- Please click on the registration button and complete the form. The form is short and completing it out should not take more than a couple of minutes. We recommend that you have your insurance card at hand.
- A patient care coordinator will contact you within 24 hours after you submit the registration form to schedule your first appointment, review our evaluation process and setup your insurance billing.
- You will receive an email confirmation of your appointment including a link to a behavioral health questionnaire. Please complete and submit this questionnaire prior to your first appointment. The questionnaire is three pages long and should take between 10-15 minutes to complete.
If you need to cancel or reschedule your appointment please do so 48 hours prior to the appointment, to avoid being charged a missed appointment fee. Notice that missed appointments are not covered by insurance. As a courtesy we email or text an appointment reminder to assist your scheduling.
